SUBJECT: Commitment to Good Behavior and Performance

On March 3 and again on April 27, we had a discussion regarding your inability to work effectively with another employee. I explained that your lack of cooperation and involvement in team activities due to feelings of anger and resentment were causing a problem and I asked you to please be more cooperative and responsive. During our meeting you promised that in the future, you would not allow your feelings and your opinions of others prevent you from contributing to the best of your ability to the efforts of the work group. But since that time I have seen little evidence of your commitment. In the past few days alone you have avoided contact with me and other members of the work team and refused to attend or fully participate in group meetings.

This is a serious situation because your behavior has interfered with the smooth operation of the work unit. Specifically, your unwillingness to share information regarding production schedules resulted in our having to postpone an important project.

In view of these facts, I am asking that you give serious consideration to your continued employment with this organization. I hope that you will decide to stay. However, if you choose to continue your employment with us, you must agree to abide by ALL of the organization’s rules and standards of conduct.

By signing below, you acknowledge that you understand the seriousness of the situation and that you agree to these conditions.

I have given serious consideration to the situation described above and wish to continue my employment with the organization. I agree to abide by all rules and standards of conduct in the organization and understand that failure to do so may result in my being discharged without any further notice or warning.
